TEAM RESERVATION FORM
DEPOSIT REQUIRED. A $500 deposit is required to hold a time slot for your team. Your time slot is not saved until your check is received and clears the bank. Your check must be made payable to NORA LEON and be mailed to 12693 BUCHANAN STREET, GRAND HAVEN, MI 49417-9612 U.S.A. The deposit will be applied to your team expenses. If your team cancels...the deposit will be forfeited!
